Has the SAQ been used as part of a screening tool
    for coronary artery disease?
Would it be appropriate to use as a screening
tool for people who have not been diagnosed with
    CHD ?

Jorgia, The SAQ was not designed to make a diagnosis of CAD and would probably not serve well in that role. Its main purpose is to quantify the severity of symptoms, the functional limitations and the impairment in quality of life among those with established disease. The Rose Angina Questionnaire was designed for diagnosing coronary disease through a mailed survey in Britain. It might be a better instrument for your purposes.
