  1. Why should I register as a member of this site?

    By Registering on this site you can:

    • Participate in online discussions in our public Forums
    • Investigators and staff can gain access to study related materials
    • License one of our health-related quality-of-life instruments (KCCQ or SAQ)

    You can learn more about outcomes research and the various health-related quality-of-life instruments we develop in the public parts of our site without registering.


  2. How do I register?
    1. Click on the Signup link at the upper right corner of any page.
    2. Complete the form on the “Signup” page with the required information — your name, email address, password, and time zone.
    3. Submit the form and you will receive an email with further instructions. There could be a few minutes delay in receiving the confirmation email.
    4. Click on the link in the e-mail which will log you in to the site — evidenced by your name appearing in the upper right corner of every page.
    5. Make sure you remember your password and which email address you used to signup!

  3. What should I do if I have not received the confirmation email?

    If you do not receive the confirmation email, please check your junk folder. If you do not find our confirmation email in your junk folder, please email us.


  4. What should I do if I have forgotten my password?

    If you have successfully logged in before, but now cannot remember your password, do this:

    1. Click on the “Login” link in the upper right corner of any page.
    2. Click the “Reset Password” link you’ll see in the Login page.
    3. Enter the email address used when registering on the site into the “Email” field and you will receive a new activation email.
    4. When you receive the activation email, click on the link and you will be prompted to edit your password.

  5. How do I change my email address?

    There is a link from Your Workspace page to make this easy:

    1. Log on to the CVO website (www.cvoutcomes.org) with the email address you used to register initially.
      NOTE: You can use your old address and password even if you no longer can receive email at that address. Please do not sign up as a new user.
    2. Go to Your Workspace page; there is a link to that page at the top right corner of every page.
    3. Click on the “Manage your email address, password, name, and profile” link in the “Actions” box in the left column of your workspace page.
    4. Update your email address in the “Email Address” section.

    If you still have problems after following these instructions, email us.


  6. How do I change my password?
    1. Go to the Your Workspace page; there is a link to that page at the top right corner of every page.
    2. Click on the “Manage your email address, password, name, and profile” link in the “Actions” box in the left column of your workspace page.
    3. Update your password in the “Password” section.

  7. I’m now registered at the CV Outcomes web site, but I can’t see the group I expected to join. What’s going on?

    Group membership is managed by Group Administrators. There may be a 24-48 hour delay before you are added to a group. If you do not have access in 2 business days, ask for help contacting the group administrator or the person who told you about the group.


  8. I just received an email with a link to a page on the site, but when I click the link, I’m told that I don’t have access. What should I do?

    In order to open any document linked to the CVO website, you must be logged in to the website. Make sure that you are logged in to the CVO website (www.cvoutcomes.org). Once you login, click the link again and you should have access to the document.


  9. What is your Privacy Policy?

    We do not sell, lease, distribute, or use in any other fashion any email address or other information provided by anyone who registers at this site. Only registered users of this site will ever receive email from us, and then only as part of their use of the various forums or other group tools or in the course of managing their licensing of the various instrument-related items we offer here. Any concerns, issues, or complaints about these policies or the operation of this site should be promptly emailed to us.
